A year and a half ago, when we just launched our website, I was talking with an old friend of mine, an experienced television producer. We discussed the possibility of creating a television show about weight loss in accordance with the principles of LCHF. At first, my friend was seriously interested in the idea, but when I described to her in more detail how LCHF works and what needs to be done to lose weight, she rejected it as not suitable for the reality show format. “You see,” she explained to me, “everything is too easy and simple for you - you eat as much as you want, all kinds of delicious natural food, and you walk around all day full and satisfied. Where is the suffering? Where is the intrigue? As an example to follow, she cited the world’s most popular weight loss show, Biggest Loser: “Look how they are tortured there, what torment the participants have to go through on the way to their cherished goal, how they suffer, how they cry, how one after another they break down and are kicked out of the program - this is interesting to viewers who watch the program, sitting comfortably on the sofa, and not how people eat scrambled eggs and bacon for pleasure.”

This story very well reflects popular ideas about the process of losing weight: through thorns to the stars, no pain - no gain. And when an analogue of Biggest Loser appeared in Russia - the show “Weighted People” on STS, it was all about the same thing: about overcoming oneself through pain and tears, through extreme stress, “through I can’t.” “I don’t feel sorry for you one bit, don’t feel sorry for yourself,” the trainer harshly reprimands the exhausted participants on the exercise machines, whose reaction to ever-increasing physical activity is best expressed by the words: “It’s just hell!” In the American version, it sounds even harsher: “I don’t care if you die on this floor now, it’s better to die, but look beautiful.” In general, there is something to see.

The methodology of all these programs is quite simple: heavy, backbreaking physical activity combined with strict calorie restriction. In fact, this is the “concentration camp method.” And it works: the show's participants lose weight, transforming before the eyes of viewers from shapeless fat men into fit athletes. In theory, this should serve as an excellent motivator and proof that nothing is impossible. At least if you are ready for suffering...

But recently, more and more publications have appeared telling about what happens behind the scenes of the program and how the lives of the participants turn out after filming ends. And this doesn’t look like a success story at all.

13 out of 14 contestants have gained weight again

Information that former participants in the show eventually regain the weight lost with such difficulty has previously been leaked to the press. But now this can be considered a scientifically proven fact. In early May, The New York Times published a lengthy article on the results of a National Institutes of Health (NIH) study published in the journal Obesity. Scientists observed 14 former participants of the show for 6 years. Only one of them managed to maintain his form at the level of the program finals. Thirteen former participants have gained significant weight over the years, with five returning to their pre-weight loss levels or even exceeding them. “On average for the hospital” the numbers look like this:

  • Weight at the start of the show: 148.9 kg
  • Body Mass Index (BMI) at the start of the show: 49.5
  • Weight in the final show: 90.6 kg
  • BMI in the show finale: 30.2
  • Weight loss during the show: 58.3 kg
  • Weight 6 years after show: 131.6 kg
  • BMI 6 years after show: 43.8
  • Weight gain over 6 years: 41 kg

Of course, it's better to weigh 131.6 kg than 148.9 kg, and some experts even call such results a success - after all, former contestants managed to maintain an average weight loss of 11.9%, while most cannot keep 10%. But, on the other hand, 131.6 kg is also a lot, and according to the same NIH classification, a BMI above 40 means extreme, or morbid, obesity. It is unlikely that the Biggest Loser participants tortured themselves in order to remain in this category. And they didn’t get lazy: their average level of physical activity remained virtually unchanged.

What went wrong? Why did almost none of the participants manage to consolidate the result obtained with such difficulty? Scientists believe that the problem lies in the serious metabolic changes that occurred in the program participants.

Decreased metabolism is the first reason for weight gain

The basic metabolic rate (Resting Metabolic Rate, RMR) is the amount of energy consumed by the body at rest to maintain its own vital functions. At the start of the program, participants' RMR was approximately normal for their weight. By the end, it had dropped significantly - below what should have corresponded to their new weight. But this is normal - the decrease in basal metabolic rate as a result of fasting and exercise is a well-known fact. But what really surprised the scientists was that over the following years, their metabolism not only did not recover, but also slowed down even more. And even when the former participants of the show regained more than forty kilograms of weight, their basal metabolism was slightly lower than at the end of the show. On average, six years later, their RMR was 700 kcal/day lower than at the start of the show and 500 kcal/day lower than it should be for people their weight. In practice, this means that their body burns much less energy at rest than it should, which means that a much larger portion of calories is stored “in reserve.”

The winner of the eighth season, 46-year-old Danny Cahill, is the absolute champion of Biggest Loser for the entire existence of the program: he managed to lose as much as 108 kilograms in 7 months. His weight more than halved: from 191 to 86.6 kilograms. “I got my life back. I feel like a million dollars!” – Danny proclaimed in the finale. To achieve these results, he set a goal of achieving a daily deficit of 3,500 kcal, which would lead him to lose a pound of fat per day. He had to quit his job and turn many hours of grueling training into his main occupation.

After the show ended, Danny Cahill became an international celebrity and began a very successful career as a motivational speaker. It would seem, who, if not him, should fight with all his might to maintain the achieved result? But over the past six years, Danny has gained 47 kilograms again, and now weighs 133.8 kg with a height of 180 cm. Danny had to end his career as a speaker and return to his main job as a land surveyor. At the same time, his body uses so much less energy that in order to simply maintain his weight constant, he must consume 800 kcal per day less than a typical man of the same weight. “All my friends drink beer without gaining much weight,” says Danny Cahill. “As soon as I started drinking beer, I immediately gained 20 pounds. I thought – something is wrong here, something is wrong with my body.”

Sean Elgair, a 36-year-old pastor from North Carolina, found himself in an even worse situation. As the show progressed, he managed to reduce his weight from 201 to 131 kg. Now he weighs 204 kg and spends 458 kcal per day less than normal. “I feel like I've been given a life sentence,” Sean says.

Hormonal changes are the second reason for weight gain

Slowing your metabolism isn't the only consequence of extreme weight loss. Former participants of the show have to constantly fight bouts of uncontrollable hunger and keep themselves from compulsive overeating. “Sometimes I’ll open a bag of chips and think, I’ll eat about five of them,” says Danny Cahill, “and suddenly I’ll have an eclipse, I’ll eat the whole bag, and then I’ll sit there and think, what have I done?” Erin Egbert, the only study participant who managed to avoid weight gain, faced similar problems despite slowing her metabolism by 552 kcal per day. “People don’t understand that treats are like a drug,” she says. “A couple of treats can easily turn into a three-day binge, and that’s something I have to constantly fight.”

Scientists believe that the main reason for this behavior is a chronic decrease in the level of leptin, the hormone responsible for achieving a feeling of satiety. All participants started the show with normal leptin levels. By the end, they had almost no more of this hormone left, which made them feel constant hunger. Over time, along with the pounds, leptin returned, but only to about 60% of the level they had before participating in Biggest Loser, which explains their constant urge to eat something.

The fact that extreme weight loss methods negatively affect leptin levels is also known from earlier studies. In 2014, scientists compared the performance of Biggest Loser participants with patients who had undergone gastric bypass surgery. Both groups lost about the same amount of weight, but the study found that reality TV participants had leptin levels five times lower than surgical patients. Additionally, the Biggest Loser methods resulted in a greater slowdown in metabolism.

Leptin is not the only hormone responsible for the feeling of fullness. In 2011, Australian scientists conducted a study in which 50 overweight men and women went through a 9-week period of an extremely low-calorie diet - 550 kcal per day. A year later, they still had lower levels of five hormones that regulate hunger and higher levels of the hormone that makes us overeat. Participants reported increased appetite.

It seems that our body is trying with all its might to regain its initial weight after a sharp weight loss, saving energy on its own vital functions and sending hormonal signals to the brain that force us to eat more.

Dangerous weight loss methods

Biggest Loser's problems aren't limited to just former contestants gaining weight back. Recent media reports suggest that reality TV's methods were much more extreme and less legal than previously believed. Thus, an article published a few days ago in the New York Post talks about how trainers and doctors gave various medications to show participants, incl. and prohibited, to enhance weight loss. Everything was used - from diuretics, leading to severe dehydration, to amphetamines. In order to expel as much moisture as possible from the body, participants were forced to spin exercise bikes in a steam room or perform grueling exercises in several layers of clothing. “I was throwing up every day,” says season two contestant Suzanne Mendonca. “[Trainer] Bob Harper tells people that vomiting is good, that they should vomit so they will lose more calories.”

Other participants talk about severe psychological pressure from trainers, numerous injuries, menstrual irregularities, thyroid problems, hair loss, joint pain and other serious health problems.

In addition, the diet was much stricter than officially indicated. Show participants admit that management instructed them to tell the press that they were following official recommendations and consuming 1,500 kcal per day, but urged them to reduce their actual intake to 800 kcal or even lower. But the most surprising thing is that some of the show's participants were forced to gain more weight before the start of the season in order to make their subsequent weight loss more spectacular.

The British Guardian newspaper reveals the physically and psychologically brutal methods of the world's most popular and influential weight-loss reality show in an article with the eloquent headline: “It's a miracle no one has died yet.”

Is there a future for reality TV shows about weight loss?

Biggest Loser's prospects seem pretty dim right now. Ratings have been falling in recent years, criticism in the media is becoming increasingly harsh, and NBC has not yet confirmed the launch of the next 18th season. Former participants and scientists themselves are calling for an end to the reality show.

As Dr. Yoni Friedhoff of the University of Ottawa says, “Biggest Loser condemns participants to either a lifetime of inhumane efforts to lose weight or to regaining the excess weight. The conclusion from this is this: if you are ready to kill yourself with exercise and diet to lose weight, then you are also killing your metabolism.”

As for “Weighted People” - the localized Russian version of Biggest Loser - I have not yet been able to find a single critical article on this topic in RuNet. The reality show was nominated for the TEFI award as the best entertainment (!) program and the next season is being shown right now on STS, contributing to the spread of unrealistic and dangerous ideas about weight loss methods.

The real problem isn't the reality show itself, though—it's simply an extreme manifestation of popular beliefs about the “correct” methods of losing weight: eating less and moving more. Difficult? We must endure! It is customary to blame the owners of extra pounds for being overweight - supposedly it is their laziness and gluttony that are deposited in the form of fat folds on the sides, and in order to get rid of them, they will have to suffer and suffer for their deadly sins. However, more and more scientific evidence indicates that this path is a dead end and does not produce long-term results. And if there’s something worth changing, it’s not just the format of the television show, but the very ideas about where excess weight comes from and how you need to change your lifestyle in order to get rid of it irrevocably and without irreversible consequences for health.

A person, whose weight is usually 150 kg or more, addresses the letter to the presenter. Then Chris Powell flies to him and begins rescuing the hero. An overweight man is sent for a medical examination, while the show's team is working on converting the hero's home into a gym.

For three months, the participant receives an assignment from the presenter to lose 40-70 kilograms of weight. Three months later, a new goal appears. After nine months, the hero gets rid of excess weight, and then his sagging skin as a result of weight loss is surgically tightened. A year later we can see the same person in a completely new form, and this transformation is truly shocking.

"Those who have lost the most weight"

Two teams of fat participants are recruited. For a certain time, they train, eat right and perform various tasks. Control weigh-ins take place regularly, according to the results of which the person who lost the least weight leaves the project. At first, the competition takes place between teams, but then each participant fights only for himself, regularly studying with a coach.

Each week, the two people who have lost the least amount of weight go home. In the USA this show is extremely popular. It has been published since 2004, and for many fat people it has become a salvation and an opportunity to regain not only external attractiveness, but also physical health. There are also versions of the program in other countries - Great Britain, Australia, Russia, Ukraine. The Russian version is called “Weighted People”, the Ukrainian version is called “Weighted and Happy”.

The host of this weight loss project is Anita Tsoi, who once lost a lot of weight herself. The show started in 2015 on the Russian TV channel Domashny. The project, by the way, became a finalist in the British PromaxUK competition in the categories “Best Reality TV Promo” and “Best Wedding Promo”.

Participants in the program are married couples who have been together for at least ten years. During such a time, of course, a lot can change, including the dimensions of both partners. At the beginning of the show they show the difference between what the spouses were like before the wedding and what they are now. Now their goal is to return to their “before” parameters, in which they are helped by specialists in the field of fitness, nutrition, cosmetology, and so on. The participants have to put in a lot of effort, but having achieved results, they all claim that they are incredibly happy with them, and that along with their physical form, much in their family relationships is also changing for the better.

"I am losing weight"

For several seasons now, the popular reality show “I’m Losing Weight” has been broadcast on the Russian NTV channel. It is attended by overweight people who really want to lose weight and improve the quality of their lives, so that the lost kilograms never return.

Each episode of the program tells the story of one hero, who shares with the audience how he managed to lose weight, what he did for this, and what difficulties he had to face.

"Now you're in the know"

Fitness reality show on the BodyMania channel, which has been successfully running for the second season. The main goal of the participants is competition.
